Conference Forms
Please review all of the instructions for completing forms before starting the form process. It is required to complete an evaluation form for each day you attend the conference and a CEU attestation form BEFORE you download and print your CEU certificate.

Session Evaluation Forms
Please complete a session evaluation form for EACH DAY you attend the conference at the end of the day.
When you open the forms, scroll towards the bottom where your email address and name will be preloaded. Click "Next". It will then go through each session of the day. If you attend, click "Yes" and a series of feedback questions will populate. Complete these questions and then click "Save and Next". If you did not attend, click "No" and it will move onto the next session. After the final session click "Save and Finalize". A popup window will appear asking you to confirm you would like to submit. Press "Ok" to submit.

CEU Attestation Form
Please complete the CEU attestation form when you have completed your LAST DAY of the conference.
When you open the form, complete your information on the first page and click "Save and Finalize". On the next pages, please enter your initials next to any sessions that you attended. At the bottom of the page, enter the total number of CEUs you earned. If you did not attend any sessions, please enter 0. Please enter information for all three days of the conference (even if you did not attend all three days). On the final page, your CEU total will be scored and you can submit.
Please print/save a copy of this form. To print/save a copy, look at your menu on the left of the screen. You will see "My Applications" and below that "Complete". Information on forms you have completed will appear in the main screen. Find that Attestation form and you will see options on the right side. Click "print" and the form will open in pdf in another window. You can print or save a copy from this screen.
CEU Certicifcate
Once you have completed your evalution forms for each day you attended the conference and completed the CEU attestation form you can print/save a copy of the CEU certificate. Click the link the below to open the form in another window where you can print/save a copy.
